Public/private partnership wins NHS award

A partnership between South Manchester Primary Care Trust (PCT) and Bupa's Ringway Mews Nursing Home in Manchester has won an award for helping to prevent residents needing hospital treatment. The Ringway Mews Active Case Management has been awarded the Adult Community Services category of the Greater Manchester Healthcare NHS Awards 2005, which recognise innovation, achievement and excellence in healthcare.

The project team proactively reviews care plans, reduces the need for admission to hospital, reduces pressure on the NHS and gives patients direct and planned access to a range of health care that they did not previously receive. The team includes an NHS doctor, pharmacist and nurse who work closely with the care home's nurses and carers.

Amanda Lindop, senior sister at Bupa's Ringway Mews Nursing Home, said: "By making regular assessments we hope to pick up on and treat potential problems at an early stage, such as a chest infection, which could result in a resident needing to see a GP or being admitted to hospital. Residents were already receiving high standards of medical care at the home but this gives added security. The project has also improved the way we manage long-term conditions. For example, the team has been given advice on how to control a resident's diabetes even more effectively."

Dr Martin Vernon, NHS doctor, said: "We are helping to improve the quality of medical care for residents rather than them having to come to an out-patient clinic or to GPs."
